A patient just asked me whether I had a theory about the cause of CFS. I do have one, and I told him, so I thought I would also share, for what it is worth:
I think Rich Vank konynenburg's theory is right in that, it is a low GSH (caused by any kind of stressor/s) what finally causes a permanent block in the methylation cycle. Your detox system fails, and also you cannot make enough DNA for your enterocytes and white cells to replicate as needed. Then the toxins load increases blocking even further this cycle (by inhibiting key enzymes), and dangerous infections take place (Borrelia, Bartonalla, HHV-6...), that in turn lowers GSH even further and weakens your immune system even more... (Note that for instance Borrelia has shown to lower GSH...).
Methylation also silences genes, including retroviruses. I think low methylation may allow for an improper expression of HERVS proteins that could be involved in an autoimmune process (recently proposed by Dr. De Meirleir as the possible etiopathogenic process behind CFS --This is quite knew so we'll see about this...).
Now, your redox status is wrecked (impared methylation leads to low BH4, that in turn leads to superOxide and peroxynitrites when used by the enzyme NOS), your immune system is down, your detoxification system fails... Finally, and because of many factors, including free radicals that impairs both the Kreb cycle and the electron transport chain, and a lack of CoQ10 or L-Carnitine (both need methylation to be synthesized), mitochondrion are affected (switching to anaerobic respiration), ATP goes down, and from here you can explain CFS (adrenal fatigue, brain fog, Gi problems, Dyastolic failure, etc, etc, etc.)
